Obtain a health history on a family member or friend as a patient and answer the following questions related to your patient. Use your textbook as a reference as needed.

Based on your patient’s age what are the most common risk factors for their age group?

Identify the patient’s internal and external variables that could influence your patient’s Health and health practices.

Identify what changes the patient should make to adopt a more healthy lifestyle?

What could you do as a nurse to encourage behavior changes in your patient?

What should you include in your plan for the patient?

What level of prevention do your plans fall under? If you have multiple, explain each.

What risk factors can you identify that increase the patient’s susceptibility to disease or increased complications with an actual disease that the patient has?

What questions did you ask to determine all of the patient’s risk factors?

What is the impact (possible impact) of the patient’s disease on their family members? (If the patient does not have a disease use the most common risk factor as an example, how would it impact their family)
